RN Case Manager – FT

Responsibilities: Performs the initial assessment, develops and implements the plan of care in coordination with physician, coordinates care with other disciplines, performs discharge planning. Has good understanding of Medicare regulations and eligibility requirements for home care, providing and documenting care for an assigned caseload of patients.

Qualifications:

  • At least one year experience as a Registered Nurse in a clinical care setting, home health preferred.
  • Must be proficient in various computer programs, preferably with Home Care software system for patient care documentation.
  • Possesses excellent coordination, organizational, and communication skills.
  • Current CPR certification required

Human Resource Manager

Northeast Professional Home Care – North Canton, Ohio
Full-time, Part-time
Northeast Professional Home Care is a locally owned home health agency that has been providing home care services to the Northeast Ohio Region for over 20 years. We have an immediate opening for an experience Human Resources Manager. Experience in home health care is a plus.

 

ESSENTIAL DUTIES:
The following statements reflect the general responsibilities and standards associated with the performance of this job. This description is not intended and shall not be interpreted as a comprehensive list of all tasks, duties, responsibilities, and skills required.

  • Manages human resource related issues
  • Develops and implements HR-related policies and procedures at the facility level. Oversees rollout and execution of Corporate HR-related policies, procedures and communications.
  • Ensures legal compliance on HR-related issues including ADA, EEO/AA, FMLA, FLSA, COBRA, and harassment.
  • Serves as a business partner to leadership team.
  • Works with the Administrator on development of job descriptions pay ranges for positions.
  • Conducts recruitment effort for all personnel and participate in the interviews;
  • Conducts new hire orientations/onboarding
  • Works directly to counsel managers and employees on employee relations and performance management situations; conduct exit interviews;
  • Performs benefit administration to include claim resolution, change reporting and communicating benefit information to employees;
  • Maintains compliance with federal and state regulations concerning employment
  • Maintains knowledge of industry trends and employment legislation
  • Administration of worker compensation reports, record keeping, documentation and hearing compliance.
  • Administration of unemployment compensation reports, record keeping, documentation and hearing compliance.
  • Participates on committees and special projects
  • Performs other incidental and related duties as required and assigned

 

ADDITIONAL RESPONSIBILITIES:

  • Attends NEPH events as necessary.
  • Maintains flexibility in work schedule to accommodate the needs of employees and prospective employees

 

EDUCATION/TRAINING/EXPERIENCE :

  • Bachelor’s Degree, preferable in Human Resources or related field;
  • PHR Certification preferred, but not required.
  • Three to five years of broad HR management experience
  • Outstanding employee relations skills & exhibits a leadership profile
  • In depth understanding of state and federal employment regulations
  • Strong analytical and problem solving skills, with the ability to resolve problems and make decisions independently
  • Experience in health care, specifically home health, desired
  • Proficient in Excel and Word, Microsoft Applications, and HRIS systems
