Our home care services promote recovery after hospitalization, surgery, or a relapse of illness. We are also dedicated to improving our patients’ quality of life when living with a chronic illness. Our goals are to provide care and in-depth education designed to prevent unnecessary rehospitalizations, emergency room visits, and medication errors through personalized services and unique care plans designed to meet your needs. Skilled Home Health services include:

Skilled Nursing Services:

Our licensed nurses perform a variety of specialized and highly skilled services for our patients. Our nurses advise and teach the patient and their family members, coordinate the treatment plan with other NPHC care team members (such as physical, occupational, speech therapists, home health aides and medical social worker), and communicate with the patient’s physician and family.

  • Services of our licensed nurses include, among other things:
  • Assessing and observing a medical condition
  • Patient education and teaching about a medical condition, such as diabetes or heart failure
  • Evaluating the response to a new medicine or to a change in medicine dosage
  • Teaching a new procedure such as an IV or dressing change
  • Performing complicated procedures such as a urinary catheter insertion and care
  • Injections
  • In-home coagulation therapy monitoring
  • I.V. Therapy
  • Patient education and teaching
  • Pain management
  • Wound care

Rehabilitation Therapy Services:

Whether recovering from surgery or short-term illness, returning home after a knee or hip replacement, or living at home with chronic illness or a disability, our rehabilitation therapy services can help.

We work closely with the patient, their caregivers and their personal physician to develop a customized care plan that focuses on patient safety, regaining mobility and maintaining independence. Our program emphasizes continuity of care and provides a smooth transition from the hospital or rehabilitation facility to home. Each patient will receive a complete assessment to determine appropriate care and therapy needs

Types of Rehabilitation Therapy:

Physical Therapy

Physical therapists treat patients who have injuries or health related problems that limit their ability to move. They are trained to help you improve function and mobility, relieve pain, and prevent or limit permanent physical disabilities. You may receive the services of a physical therapist if you have:

  • Joint replacements
  • Cardio-pulmonary disease
  • Stroke or neurological disorders or diseases
  • Fall risk

 

Speech Therapy

Speech therapists help patients experiencing problems with cognition, speech, language, voice, or swallowing due to conditions such as stroke, head injury, Parkinson’s disease or dementia. They will evaluate the patient and develop an individualized treatment plan to improve breathing, speaking and/or swallowing function.

Occupational Therapy

A licensed occupational therapist, in conjunction with other members of the home care team, will develop and implement an in-home treatment plan to retrain and rehabilitate essential life skills lost as a result of an illness, surgery or injury. This includes helping our patients achieve the highest level of independence in dressing, bathing, showering, feeding, and home/kitchen management/meal preparation. An Occupational Therapy evaluation looks at all aspects of the client: cognition, vision, sensation, strength, mobility as well as their roles in the home and their goals for returning to their previous functional level. You may receive the services of an occupational therapist if you have:

  • Diabetes
  • Heart Failure
  • Chronic Obstructive Pulmonary Disease
  • Cognitive and Behavioral Health Conditions such as Alzheimer’s’ Disease

Through quality home care and compassionate services, our staff members help ease the burden of those having difficulty with Activities of Daily Living (also known as “ADLs”). We are proud to help our clients to live in their homes longer, as safe and independent as possible, providing high levels of care, all in the home.
