Home care aide

What is the role of a home care aide?

Home care aides (also known as home health aides) are important members of the care team who work under the supervision of skilled nurses to support patients with activities of daily living (ADLs), monitoring of their health condition, meal planning and preparation, light housekeeping, errands, and companionship.

Our home care aides serve as a primary partner with patients in achieving your healthcare plan and recovery goals and helping you maintain independence.

Our staff works directly with your case manager, physician, and family to develop a personalized plan for your care and recovery, in the comfort and privacy of your own familiar surroundings.

Services of our home care aides include:

  • Assisting with daily living activities, such as bathing, dressing, meal planning and preparation, and mobility
  • Monitoring patient’s vital signs and physical and mental condition
  • Running errands, like grocery shopping, filling medication prescriptions, picking up drycleaning
  • Providing companionship to our clients and helping facilitate entertainment activities and hobbies
  • Taking action during emergency health situations

Home care aide support can be provided on a short-term or long-term basis and is often covered by PASSPORT, Medicaid, Medicaid Waiver/MyCare, or Self-Pay.

What makes someone a candidate for home care aide services:

  • Home care aides work alongside our care team to ensure you maintain independence and as high a quality of life as possible. If you’re receiving in-home healthcare from a medical team, a home care aide can help provide personalized, one-on-one support so you can continue to participate in activities that bring more joy to your life.
